The Mark of Quality that Sets Your Medical Home Above the Rest

 

The Accreditation Association for Ambulatory Health Care is the nation’s foremost ambulatory health care accrediting organization. We are the only one to offer on-site accreditation surveys for organizations seeking Medical Home accreditation. 

 

Comprehensive in Scope

AAAHC accreditation consists of a survey of your staff, your facility, your equipment, your medical procedures and the coordination of your care procedures. For your organization and your staff, it is a symbol of prestige that you can be truly proud of; a mark of excellence honoring the fact that you adopt best practices and comply with nationally recognized standards of care. For your patients, it provides comforting reassurance that they will receive high-quality care when they need it.

 

The AAAHC Handbook Provides Specific Standards

In the AAAHC Accreditation Handbook for Ambulatory Health Care, we provide specific standards for the Medical Home. The standards specify that an accredited Medical Home is patient-centered, physician-directed, comprehensive, accessible, provides for continuity of care, and is organized to meet the needs of the individual patient.  

 

When you seek AAAHC accreditation as a Medical Home, you demonstrate your commitment to high standards, and you are assessed on the following characteristics:

 

·      Relationship with the patient and the patient’s family and caretakers, and members of the Medical Home health care team

·      Continuity of care including documentation of all consultations and appointments and proactively planned transitions of care

·      Comprehensiveness of care including preventive and wellness care, acute care, chronic illness management and end-of-life care

·      Accessibility of care. Patients are provided information about how to obtain medical care at any time, 24/7, 365 days a year

·      Quality, physician-directed care and periodic assessment of evidence-based guidelines and performance measures

·      Electronic data management is continually assessed as a tool for facilitating the above-mentioned standardsome

 

AAAHC surveys are unique in that they are peer-based; conducted by professionals who are experienced ambulatory health care providers.  During the actual survey, we work with you collaboratively and consultatively to assure that your patients receive high-quality care consistent with AAAHC standards for the ideal Medical Home model.
