We offer two ways to Medical Home recognition: On-site certification and full accreditation.

With almost 5,000 organizations accredited, AAAHC is the nation's leading ambulatory health care accrediting organization. We're also the only accrediting body offering two ways to attain Medical Home recognition. 

1. AAAHC MEDICAL HOME ON-SITE CERTIFICATION

As an alternative to our full AAAHC accreditation, you can now achieve Medical Home On-Site Certification. Our Medical Home On-Site Certification includes pre- and post-survey consultation and an on-site visit provided by an experienced AAAHC surveyor.

The AAAHC Medical Home On-site Certification Handbook describe the principles of a patient-centered Medical Home as, physician-, nurse practioner-, or assistant-directed, focused on the patient-provider relationship, comprehensive, accessible, and providing for continuity of care.

2. AAAHC MEDICAL HOME ACCREDITATION

Think of AAAHC Medical Home accreditation as the deluxe version - the regular AAAHC accreditation process, reviewing your organization, your staff, your facility, your equipment, your medical procedures, and the coordination of your care procedures, plus Chapter 27 of the AAAHC Handbook for Ambulatory Health Care on Medical Home. It represents the most comprehensive Medical Home survey process available, conducted at your facility by experienced AAAHC surveyors.
